Lost, Stolen, or Damaged Token

Problem

"I lost the hardware token that I received from the Service Desk. How do I authenticate?"

Solution

  1. Call the ITS Service Desk to report your token as lost, stolen, or damaged.

  2. At the same time, request a bypass code to use to authenticate.

  3. Submit a Duo Hardware Token Request. Once your request is approved, your token can be picked up at the Service Desk

There is a $20 fee for replacement tokens. The fee is charged through your Campus Express Account
