Forms in Microsoft 365 - QuickStart

Forms in Microsoft 365 can be used to create surveys, questionnaires, quizzes, or other forms that can be easily shared.

1. Log in to the My Cal Poly Portal > My Apps > Email & Calendar. 

2. In the upper left-hand corner of the page, click on the app launcher

   . 

3. Click on Forms.
