If you reset your phone to factory settings or replaced it with a new mobile phone but kept the same phone number, you may need to reactivate Duo Mobile.
Step 1 - Authenticate

Log in to the My Cal Poly portal.
Go to the Personal Info tab > Personal Info > Authorized Devices > Multi-factor Authentication > click Edit.
Click Other Options.
You’ll be prompted to authenticate now, do not respond to it. Wait until step #6 to authenticate instead.
Select Manage devices. This is located at the bottom of the list.
Choose the phone number you wish to reactivate from the drop-down list, then select an authentication method and complete the verification on your currently enrolled device.
Once authenticated, you will see a page that lists your devices in square cells.
Step 2 - Reactivate

Click I have a new phone. (contd from Step 1 above)
Click the Get started button.
Complete the re-enrollment by inputting your information following the instructions presented in the corresponding prompts. See details in Step 3 below…
Step 3 - Install Duo Mobile App & Add Device

Download the Duo Mobile app on your mobile device, from the App Store or Google Play. Then, in continuation from Step 2 above, click Next.
On your mobile phone, at the top right of the screen, tap the Add + button to add an account.
Tap Use QR code. You may need to allow access to the camera on your phone.
Using the Duo app camera preview screen that is displayed on your phone, point your phone at the QR code on your computer screen to complete the activation.
The QR Code will show greyed with a check when done successfully.
Click the Continue button to complete the process.
On your phone, name the account and tap Save. Your account is now linked. Click Practice now to learn how to use Duo and follow the prompts. When asked, tap the blue Allow notifications button to receive notifications, then the Ok button.
