Problem
"I lost my phone. What do I do? How do I authenticate?"

Log in to the My Cal Poly Portal.
Go to Click the Personal Info tab > >Under Personal Info portlet Info > Authorized Devices > Multi-factor Authentication > Click Edit.
Authenticate with your backup device (a secondary mobile device like a tablet, landline, or token).
Next to the device name, click the Device Options button. Delete the device by clicking the red Trash button, then the red Remove button.
