You have never had a PolyCard in the past. 

1. Upload a photo of yourself via the CS Gold WebCard Center. 
2. Verify the address where you'd like the card sent. Go to the My Cal Poly Portal > Personal Info tab > Personal Info > My Info portlet > Mailing Address.
   1. If the mailing address is correct, you don't have to do anything else.
   2. If the mailing address is not correct, you need to update it in the My Cal Poly Portal. 
3. Upload a photo of yourself via the CS Gold WebCard Center. 
4. Once your photo is approved, your card will be mailed to you within 5 - 7 business days.

2. Fill out the PolyCard Replacement Request Form.
3. If you want to change your picture, upload a photo of yourself via the CS Gold WebCard Center.
