If you reset your phone to factory settings or replaced it with a new mobile phone but kept the same phone number, you may need to reactivate Duo Mobile. For example, your iOS device might ask you to Reconnect. Tap Reconnect, then follow these instructions.
Step 1 - Authenticate

Log in to the My Cal Poly portalPortal. Go to Click the Personal Info tab > Personal Info portlet > Authorized Devices > Info tab. Under Personal Info, select Authorized Devices. Open the Multi-factor Authentication > dropdown. Click Edit.

Select the type of device you are adding. Click the Continue button.

Install Duo Mobile on your phone. Image Removed On your mobile phone, launch the App Store app and search for Duo Mobile. Tap Get, Install to download the app, then Open. Image Removed Tap Allow to allow Duo Mobile to send you notifications. Image Removed On your computer, click the I have Duo Mobile installed button. Image Removed On your mobile phone, at the top right of the screen, tap the + button to add an account. Image Removed Click OK Image AddedClick Other options. Image AddedYou’ll be prompted to authenticate now, but do not respond to the prompt. Wait until Step 6 to authenticate instead. Select Manage devices. This tab is located at the bottom of the list. Image AddedChoose the phone number you wish to reactivate from the dropdown list, then select an authentication method. Complete the verification on your currently enrolled device. Step 3 - Install Duo Mobile App & Add Device

Download the Duo Mobile app from the App Store or Google Play on your mobile device. Click Next. Image AddedOn your mobile phone, tap the Add button at the top right of the screen to add an account.
Image AddedTap Use QR code. You may need to allow access to the camera on your phone.
Image Removed On your computer, scan the barcode to activate Duo Mobile. Image Removed You are now activated. Your device is ready to approve Duo authentication requests. Click the Continue button to add a second device. Image Removed- Image Added
Using the Duo app's camera preview screen displayed on your phone, point your phone at the QR code on your computer screen to complete the activation. When done successfully, the QR Code will become grayed out with a green check. Image AddedClick Continue to complete the process. Image AddedOn your phone, name the account and tap Save. Your account is now linked. Click Practice now to learn how to use Duo and follow the prompts. When asked, tap the blue Allow notifications button to receive notifications, then the Ok button.
