You had a PolyCard in the past , but no longer have one.
- Fill out the PolyCard Replacement Request Form.
- If you want to change your picture, upload a photo of yourself via the CS Gold WebCard Center.
- Verify the address where you'd like the card sent. Go to the My Cal Poly Portal > Personal Info tab > Personal Info > My Info portlet > Mailing Address.
- If the mailing address is correct, you don't have to do anything else. Your PolyCard will be mailed within 5 - 7 business days of your request being approved.
- If the mailing address is not correct, you need to update it in the My Cal Poly Portal. Your PolyCard will be mailed within 5 - 7 business days of your request being approved.
